If Contracts are to be used on a regular basis to improve a specific behaviour then to save time, it is recommended that practitioners create their own blank template (in a Word document) which enables the details to be entered in before printing off copies to be signed. See example

Explanation
1) This Contract forms an agreement between the client (Name) and the practitioner (Name) and the supervisor (Name) (if appropriate) with regards to the conditions as specified below. Failure to comply with these conditions will lead to the termination of this contract and alternative action may have to be considered.

Purpose
The purpose of this contract is to (List the scope, identify all tasks, work elements, objectives of the contract and timetable by which the tasks must be completed).
I agree to meet to review progress (Time & Location)

Term and Conditions of the Contract
Any difficulties in fulfilling the terms of the Contract must be discussed with the supervisors as early as possible.
